FWIW - the problem with my first girlfriend was we were 19 and 20 and barely knew what we were doing. Add into that her ongoing guilt about how her Calvinist grandmother who raised her would be soooooo disappointed in her (she would starting crying after she came), and the condom issues were just the perfect cherry on top.
