So itâs hard to explain this without betting into the weeds too much here, but when you put someone on a breathing machine, you fundamentally change the mechanics of how they breathe.
For normal people, we create negative pressure to breathe. Our diaphragm opens our lungs and air rushes in. For people on a breathing machine, the machine creates positive pressure to push the lungs open. We believe that this has significant consequences physiologically in several ways.
- COVID makes the lung swiss cheese and requires big pressures to open the lung, this can cause further damage.
- Being on a vent requires large amounts of sedation, which keeps people still which has further negative consequences including more clots, less movement, not being able to eat, etc.
- The positive pressure lowers blood pressure and can cause some issues
You can get much deeper than this, but thatâs the gist of it.

Are #2 and #3 specific to COVID?
For example if you took 1000 people with bad colds and stuck 500 of them on vents at the first sign of symptoms, there would probably be more problems with the vent group because of the general risks of being on a vent. However, with COVID it seems like itâs more than that, which is explained by your #1.
If itâs the positive pressure that is so bad, maybe we should bring back iron lungs?

#3 is a physiologic truth. Increased pressure in the chest causes decreased venous blood return to the heart, which causes lower blood pressure.
#2 seemed to be true, but thatâs from anecdotal experience and not something I could cite.

In an odd way, this helped me understand my own momâs passing. Her lungs failed and they had to put her on a vent for a week. Now I understand why the doc explained her lungs would probably never work on their own again. They eventually took her off with our approval per her DNR, and just as they said, once off the vent, she simply stopped breathing.

The truth is the Lincoln Project is pushing buttons and agitating TRumps in ways the left simply never will. Just because one might tip their hat to them doesnât mean they condone their political lives nor does it mean anyone wants them at the table.
It would be great if ANYONE left or center could be even a little aggressive during campaign season but it seems mostly impossible.
